If it is an Emergency please dial 911 or go to the nearest Emergency room. For a nurse please call National Nurse Advice Line 800-874-2273 option 1.
Can I drop off my child’s physical form to be completed without an appointment?
Yes forms can be dropped off If your child has an updated well visit. EFMP/Overseas Screening may require additional visit or information.
My provider has put in a referral for my child what’s next?
Please call 910-450-4357 after 48–72 working hours to activate your referral.
