Occupational Health

First and foremost, always ensure that you and your co-workers are safe and notify your supervisor of all work-related injuries or unsafe working conditions. 

IN THE EVENT OF AN INJURY AT THE WORKPLACE:
•    Injured employees may seek treatment at a facility of their choice.
•    For emergencies, please call 911.
•    For injuries needed immediate or urgent treatment, please proceed to an emergency room or urgent care facility. 
•    For minor injuries, employees or supervisors may call the Occupational Health Clinic to schedule an appointment. The Occupational Health Clinic is unable to see walk-ins. Prior to the scheduled appointment, the employee and the supervisor must complete the Occupational Health Permit and bring it to the clinic. 
•    For every scheduled follow-up appointment, the employee must bring in a new Occupational Health Permit. This ensures accurate communication between the employee, supervisor, and the clinic for the status of the employee. 

•    All supervisors must submit an electronic Medical Surveillance Form (MSF) through e-mail for all employees that require work-related certification or surveillance exams.
•    Please see MSF Go-By Instructions for help filling out the form correctly. 
•    In the Global Address List (GAL), search for us by name (DHA Camp Lejeune Camp Lejeune NMC List NMCCL Occ Health Staff) or copy and paste our email (dha.lejeune.camp-lejeune-nmc.list.nmccl-occ-health-staff@health.mil). 
•    We will then book the appointment and send the appointment notification back to the supervisor and safety representative that are identified on the MSF.
